Job description

Heritage Hospice is excited to resume services in the Albuquerque community. Our dedicated team is hiring and ready to provide compassionate, highquality care for patients and families across the region.

The PRN Hospice Registered Nurse provides compassionate, skilled nursing care to patients and families facing lifelimiting illness. This role supports the interdisciplinary team by performing patient assessments, delivering symptom management, and ensuring highquality, patientcentered care across homes, assisted living communities, and skilled nursing facilities. The PRN RN offers flexible coverage for visits, oncall shifts, admissions, and routine support based on patient and team needs.

Key Responsibilities

  • Provide direct nursing care, including assessments, symptom management, medications, and patient/family support
  • Perform PRN visits, admissions, followup visits, and urgent response visits as assigned
  • Collaborate with the interdisciplinary team (IDG) to support highquality care plans
  • Document all care and communication accurately and timely according to regulatory requirements
  • Educate patients, families, and caregivers on disease processes, comfort measures, and hospice philosophy
  • Communicate changes in patient condition promptly to the RN Case Manager or clinical leadership
  • Participate in oncall rotations as available (if part of PRN expectations)
  • Ensure a safe and supportive environment in patient homes or contracted facilities
  • Adhere to all CMS, state, and hospice organizational standards of practice

Qualifications

  • Current RN license in the state of New Mexico
  • Minimum 1 year of nursing experience; hospice, palliative care, home health, or geriatric experience preferred
  • Strong assessment, prioritization, and criticalthinking skills
  • Compassionate communication and ability to support families during endoflife transitions
  • Valid driver's license, reliable transportation, and proof of auto insurance
  • Ability to work independently in field settings with professionalism and integrity
