Job description

At the University of Rochester Medicine Home Healthcare, we understand the heart and skill it takes to provide end-of-life care. That’s why we offer a supportive, flexible work environment that values your expertise, compassion, and time.

We are currently looking for a Hospice RN – Weekends to join our team. This is a 30/40 position, meaning you will work 30 hours per week—primarily over the weekend—but receive pay and benefits equivalent to a full-time 40-hour role. This position is ideal for nurses seeking a condensed schedule while maintaining full-time compensation.

Schedule: 3x10's, 8am to 6pm Fri- Sun OR Sat- Mon

Qualifications:

  • New York State licensed Registered Nurse
  • ASN with 1 year of acute care or Home care experience
  • or BSN with 6 months of acute/ home care expereince
  • Hospice experience preferred.
  • Excellent communication (oral and written) together with organizational and interpersonal skills.
  • E-Record (electronic medical record systems)
  • Current NYS Driver’s License

The Hospice RN will provide:

  • Ongoing assessment of patients, ensuring appropriate initiation of care plans, services/levels, and assessment information that accurately reflects the patient’s clinical, functional, and psychosocial needs.
  • Provides education to patients, families, and providers regarding the hospice philosophy. Evaluates patients for appropriate levels of care and admits patients for hospice care services.
  • Facilitation of medication, supply, and equipment orders according to patient/family needs.

Physical Requirements: The position involves frequent sitting, standing, and walking. Occasional bending and pushing may be required. Tasks such as kneeling, squatting, reaching, twisting, balancing, climbing, and pulling are required infrequently.
