About the Role

Looking for more than a med pass? Horizon House is looking for a reliable and compassionate LPN or RN who can provide hands-on care today and grow into a clinical leadership role as we transition to a Resident Care Coordinator (RCC) model.

Starting Pay Rate (depending on prior experience with license):

  • LPN: $40.00 – $45.00 per hour
  • RN: $46.00 – $50.00 per hour

Shift Differentials:

  • Evening: +$1.90 | Night: +$2.75
  • Weekend Day: +$2.75 | Weekend Eve: +$4.00 | Weekend Night: +$4.75

What You’ll Do

Start as a Floor Nurse:

  • Administer medications and treatments
  • Monitor and respond to changes in condition
  • Document in EHR (PointClickCare)
  • Collaborate with NACs and care team
  • Provide shift-based leadership by guiding, supporting, and holding care staff accountable to care standards
  • Ensure care is completed safely, timely, and in alignment with resident plans and community expectations

Floor Nurse Schedule:

  • 7 days on, days 7 off (Wednesday through Tuesday)
  • Option 1: 10:30pm to 8:30am (Memory Care)
  • Option 2: 9:00pm to 8:00am

Grow into RCC (Resident Care Coordinator):

  • Lead admissions, annual, and change-of-condition assessments
  • Oversee and support caregivers and med techs
  • Ensure care plans are accurate and followed
  • Communicate with families and providers
  • Provide clear direction, coaching, and real-time feedback to staff during the shift
  • Promote accountability and consistency in care delivery across the team
  • Act as a clinical leader on the floor

RCC Schedule:

2-week rotation, 7:00pm to 8:00am, every other weekend off

What We’re Looking For

  • Active WA State LPN or RN license
  • Strong medication administration and assessment skills
  • Ability to guide and support care staff in a team environment
  • Clear communication and confidence in addressing concerns in real time
  • Comfortable with change and growth
  • Assisted Living, LTC, or geriatric experience preferred
