Job description

Job Title

Utilizes a multi-disciplinary plan of care to manage a defined population of patients/clients providing coordination of patient care which meets IHS's standards of quality, efficiency and desired outcomes.

Qualifications

Minimum Qualifications:

  • Associate Degree in Nursing
  • Working knowledge of acute care, rehabilitation, or home care

Licensure/Certification/Registration:

  • One of the following:
    • Current Alabama RN license
    • Current Multi State RN License in accordance with Nurse Licensure Compact (NLC) for Alabama*

    *Infirmary Health abides by the NLC requirements and guidelines for the state of Alabama

  • For specific areas CRRN (Certified Rehabilitation RN) is required (or eligibility)

Desired Qualifications:

  • BSN preferred; Master's degree in a clinical specialty desirable
  • Some background in management, clinical education or case management
  • National certification applicable to area of clinical practice
