Job description

Position Summary:

The Case Manager, in collaboration with the physician,providesindividual program management for each patient toensure the patient's progression through the continuum of care in a manner that achieves the desired clinical andfinancial outcomes. Monitors and manages clinical and financial coordination of treatmentplanof assigned patientsto ensuretimely, cost-effective, individualized service delivery. Works with rehabilitation patients with variousdisabilities including, but not limitedto:spinal cord injury, brain injury, cerebrovascular accident, amputation,neurologic disorders, orthopedicconditions, and arthritis. Coordinates length of stay management within Medicare(CMS) guidelines and 60% compliance threshold.

Minimum Qualifications:
  • Current state licensure as a Registered Nurse, Licensed Social Worker, PT, OT, SLP.
  • Minimum of 3 years healthcare and clinical experience in a hospital setting.
Desired Qualifications:
  • 1 yearmedical rehabilitation experience.
  • Certification in case management preferred.
Knowledge,Skillsand Ability Requirements:
  • Excellent communication, negotiation, and conflict resolution skillsrequired.
  • Knowledge of reimbursement systems preferred.
