As a Care Coordinator (Rehab), you'll play an essential role in each patient's recovery by delivering provider-directed therapies, helping patients feel comfortable throughout their treatment, and ensuring every step of their care is completed with accuracy, efficiency, and compassion.

What You'll Do
  • Deliver provider-directed rehabilitation therapies safely and accurately.

  • Prepare patients for therapy and explain what to expect.

  • Help patients stay on track with their prescribed treatment plans.

  • Document therapies accurately in the electronic medical record (EHR).

  • Educate patients on therapy exercises and the proper use of Durable Medical Equipment (DME).

  • Monitor patient progress and communicate concerns to providers when appropriate.

  • Verify insurance information and support authorization workflows.

  • Coordinate closely with providers and teammates to deliver a seamless patient experience.

  • Maintain an organized treatment area while supporting efficient daily clinic operations.
