Who We Are

Wee Care offers Therapy Services and Home and Community-Based Services using evidence-based treatment models to help your loved one achieve and maintain the highest level of independence. Wee Care has been serving families in our community and surrounding communities since April 2006.

Role Responsibilities

The HCBS Care Coordinator is the central point of contact for assigned members and providers, helping ensure dependable, high-quality Home and Community-Based Services from intake through ongoing coordination.
In this role, you will:
- Organize and complete intake steps and related follow-up
- Participate in planning meetings and promptly capture habilitation goals and tasks
- Maintain timely, accurate documentation of service activity and communications
- Support provider orientation and re-orientation as needed
- Monitor member and provider needs, escalate concerns appropriately, and help remove barriers to continuity
- Complete outreach and follow-through that supports retention and uninterrupted services
Required qualifications/skills:
- Direct Support Provider experience (required)
- Experience in care coordination, case management, human services, or related work
- Strong organization, responsiveness, attention to detail, and consistent follow-through
- Clear written and verbal communication; able to work independently and collaboratively
- Bilingual English/Spanish (preferred)
