Help individuals and families build confidence, skills, and stability as a Self-Sufficiency Case Manager. Join our team and be a source of advocacy and support!
WHAT YOU'LL DO:
Empower & Support:Provide case management services that help individuals and families set and achieve personal goals.
Guide Participants:Conduct assessments, develop individualized plans, and assist in stabilizing housing situations.
Manage Cases: Keep detailed records, document all contacts, and maintain participant confidentiality.
Promote Self-Sufficiency: Support participants in becoming independent from public assistance.
Community Engagement: Build relationships with service providers, attend networking meetings, and advocate for participants.
Stay Organized: Ensure accurate documentation, track referrals, and complete necessary reports on time.
SKILLS TO WOW US:
You're a strong communicator, both written and verbal, who easily connects with people.
You can assess needs and create action plans that drive real results.
You have experience working with multiple population types and advocating for individuals.
Problem-solving and conflict resolution? You handle them with confidence.
You're a team player who also thrives when working independently.
You approach cultural and economic differences with sensitivity and understanding.
EDUCATION AND EXPERIENCE:
Minimum of two (2) - four (4) years of education and/or experience in a related field required.
A four-year post-secondary degree in Human Services in preferred.
