Description
Job Purpose:
Under the supervision of the Mental Health Program Manager, the CMHC provides clerical/clinical coordination with multiple agencies and providers in the community in order to connect patients with appropriate Behavioral Health care and resources programs.
Duties/Responsibilities:
• This position will be the primary liaison between individuals and organizations who provide Behavioral Health services
• Create and maintain relationships with private practice and agencies regarding counseling, psychiatric, and care coordination services across community partners.
• Represent community behavioral Health program in sponsored events and activities as applicable.
• Assist patients in referral process, maintain scheduling for multiple providers, request documents from community agencies to coordinate care, and manage feedback from patients and providers.
• Acts as the first point of contact for mental health patients and provide relief for day-to-day clerical support to administrative office activities for the behavioral health team.
• Adheres to all mental health protocols regarding the management and compassionate care of patients undergoing behavioral health crisis.
• Contribute to the creation of policies, procedures, and workflows for CMHC position.
• Demonstrate awareness and sensitivity in the delivery of activities and events as it relates to the cultural and socioeconomic characteristics of the populations served.
• Accurately determine patient needs and match those individuals to the appropriate level of care.
• Other duties as assigned.
• Ability to adapt and respond with communication strategies to stabilize, slow, or reduce the intensity of a potentially high stress situation.
Requirements
Required Skills/Abilities:
  • Excellent verbal and written communication skills.
  • Reliable attendance and consistent community engagement
  • Experience working in the Behavioral Health and Substance Use field
  • Experience with Microsoft Office Suite including Outlook, Word, Excel, and PowerPoint.
  • Experience with Electronic Health Records
  • Knowledge and experience of website management preferred.
  • Highly self-motivated and able to function independently.
  • Must be fiscally focused and mission driven.

Education and Experience:
  • Bachelor's degree in health care management/administration, business administration, public health, social work, psychology or related field, or associate's degree in similar field with 3-5 years' experience in healthcare setting.
  • Previous experience with non-profit organizations is preferred.

Immunizations Required:
  • Hepatitis B, COVID-19 Vaccination

Physical Requirements:
  • Prolonged periods sitting and/or standing at a desk working at a computer.
  • Ability to lift up to 25 pounds infrequently
