This position requires all employees have a current license with a driving record that meets the company's liability insurance policy. The following will automatically disqualify you from this requirement:

  • Driver's who are under the age of 23;
  • Driver's with any major convictions in the past 36 months (DUI, drag racing, open alcohol container, reckless driving, driving while suspended, etc.);
  • Driver's with more than three violations in the past 12 months (speeding, failure to yield, illegal passing, etc.);
  • Driver's with more than five violations in the past 24 months.
  • Driver's who have not held a valid driver's license in the US or Canada for a minimum of three years.




POSITION SUMMARY:

The Direct Care Staff at the Reach For Your Potential Day Habilitation complete a variety of tasks throughout the day. Each staff member is responsible for all of their assigned groupโ€™s needs and activities. Direct Care Staff are encouraged to work together to ensure the best services and supervision for each of the members at all times. The Direct Care Staff are what makes our agency work; they are expected to be dependable and care for the members with patience and attention to individual needs and wants.

ESSENTIAL JOB FUNCTIONS:

  • Treat members with dignity and respect at all times.
  • Maintains collaborative professional relationships with member(s), Program Director, Service Coordinator, Facilitator and RN to better serve members.
  • Facilitates the implementation of member(s) ISPโ€™s and helps members complete their goals.
  • Abides by Reach For Your Potentialโ€™s policies and procedures with a positive attitude.
  • Attend community outings and activities as scheduled. Assist with all activities of daily living, including toileting, feeding, and hygiene needs. Provide on-going supports to members.
  • Helps member(s) to advocate for themselves by gathering information, reviewing, and analyzing all aspects of an issue or concern.
  • Identifies crisis situations, discusses the incident with staff and members, and provides coping mechanisms to members.
  • Keeps up to date knowledge of memberโ€™s health, symptoms, and medications.
  • Maintains necessary training to best serve members. Assist in keeping the clean by completing both assigned and unassigned duties.
  • Documents cash tracking accurately and obtains receipts.
  • Completes documentation before the end of shift, including Edoc notes, goals, and log prompts.
  • Turns in time sheet when due and fills out accurately. Switch shifts using the appropriate document. Works scheduled shifts and arrives to shift on time. Abide by the time-off requests due date and filled out correctly.
  • Document passing medication appropriately and accurately as well as schedule II medications.
  • Attends scheduled staff meetings.
  • Treats co-staff and supervisors with dignity and respect by listening to what others have to say and receiving constructive criticism.
  • Communicates accurate and important information to direct supervisor. Communicates effectively with co-workers.
  • Drives team-related work and respects the ideas and opinions of team members.
  • Presents themselves in a professional manner.
  • Helps to welcome and train new staff.
  • Handles stressful situations and helps to problem solve. Displays an ability to learn rapidly and adapt quickly to changing situations.
  • Other duties as assigned.


JOB REQUIREMENTS:


EDUCATION/ EXPERIENCE:

High School Degree or its equivalent.


SKILLS:

Must be able to read, write and communicate effectively. Patience in working with adults with disabilities is necessary. Needs to have a caring attitude, and be caring and supportive to members. Must be able to lift at least 50 pounds and assist with daily programming needs as required. A good driving record is required.

SUPERVISORY RESPONSIBILITIES:

None.

EQUIPMENT TO BE USED:

Knowledge of how to operate typical household appliances, including stove, microwave, dishwasher, etc. is preferred. Training for specialized adaptive equipment for individual members will be provided.

TYPICAL PHYSICAL DEMANDS:

It is preferred that the employee is able to lift at least 50 lbs. Must be willing and able to assist with toileting and other activities of daily living as required.

WORKING CONDITIONS:

Majority of work is done at the Reach For Your Potential Day Habilitation. Includes assisting members to attend community outings as well. Schedules are as determined by the Day Habilitation Program Director. May be called on to work overtime or odd schedules.
