About the job:

The Registered Nurse under the direction and supervision of the Clinic Manager, provides skilled nursing care to participants in the PACE clinic and in participantโ€™s homes, implements primary care orders, evaluates participant symptoms in person and/or by telephone, develops appropriate plan of care, provides participant, caregiver and family teaching and communicates participantโ€™s condition to primary caregivers, physician and Interdisciplinary Team.

Pay: $39.00-$50.50/HR

Schedule: Monday-Friday 8:00 AM-4:30 PM. On call rotation

Required Qualifications:ย To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

  • Graduate of an accredited school of professional nursing
  • Current state Registered Nurses (R.N.) License
  • Cardiopulmonary Resuscitation (CPR) certification required
  • A minimum of one yearโ€™s experience in working with the frail or elderly population required.
  • Must have a valid driverโ€™s license, proof of insurance and have means of transportation
  • Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact

Preferred Skills:

  • Bachelors of Science Nursing (BSN) preferred.

Essentials:ย 

  • Conducts comprehensive initial nursing assessments in the PACE clinic and in participantโ€™s homes with new PACE enrollees. Coordinates with the Interdisciplinary Team to develop a comprehensive plan of care for each participant.
  • Conducts an in-person nursing re-assessments in the PACE clinic and in participantโ€™s homes semiannually and as needed.
  • Participates in the nursing on-call rotation.
  • Functions as a member of the Interdisciplinary Team. Maintains regular attendance at, and participates in Interdisciplinary Team meetings; communicates participant changes, collaborates on plan of care decisions and coordination for twenty-four (24) hour care delivery.
  • In collaboration with the Home Care Manager, completes Supervisory Visits of Home Care Aides in participantโ€™s homes per policy.
  • As needed provides orientation, initial and annual competencies of Licensed Practical Nurse (LPN), Certified Nursing Assistantโ€™s (C.N.A.โ€™s), and Clinic Support staff.
  • Performs general nursing care to participants both in the clinic and in participantโ€™s homes within the nursing scope of practice
