Case Management Supervisor - Full Time - Days - Jackson

GENERAL SUMMARY: 

Full Time Benefits Eligible

Under the direction of the Manager of Case Management, supervises functions of case management including basic discharge planning activities to achieve desired outcomes. Participates in the establishment of policies, procedures and work processes. Maintains performance improvement activities within the department and participates in the department services first goals and objectives. 

EDUCATION/EXPERIENCE REQUIRED: 

  • Bachelors degree in nursing, healthcare management or related health care field required or a Masters of Social Work required
  • Minimum of three (3) years clinical experience; hospital-based case management preferred
  • Working knowledge of Case Management. Communication and interpersonal skills. Analytical, critical thinking and problem-solving skills
  • Outcome measurement focused
  • Computer skills, including software such as Word, Word Prefect, Excel, Power Point, Visio/Flow charting
  • Positive physician and payer relationship

CERTIFICATIONS/LICENSURES REQUIRED: 

  • Full License to practice as a Registered Nurse in the State of Michigan, or Full License (LMSW) to practice as a Social Worker in the State of Michigan
  • Certification in Case Management (CCM) by the Commission for Case Management Certification (CCMC) preferred
