Role Overview
As a Hospice RN at VitalCaring, you are more than a nurse - you are the clinical leader, advocate, and steady guide for your patients and their families.
You'll lead the plan of care, manage symptoms, and ensure every patient receives personalized, comfort-focused care aligned with their goals and wishes. Through each visit, you bring not only clinical expertise, but also reassurance, clarity, and compassion.
What You'll Do
Lead Patient-Centered Hospice Care
โข Conduct comprehensive patient and family assessments
โข Develop and manage individualized, comfort-focused care plans
โข Deliver skilled nursing care with a focus on pain and symptom management
Serve as the Care Coordinator
โข Act as case manager, coordinating care across the interdisciplinary team
โข Collaborate with physicians, social workers, chaplains, and aides
โข Monitor patient condition and adjust care plans to maintain comfort
Support Patients & Families
โข Provide education, emotional support, and guidance throughout the journey
โข Help families navigate complex decisions with empathy and clarity
โข Build meaningful, trusting relationships in deeply personal moments
Drive Quality & Compliance
โข Maintain accurate, timely documentation
โข Participate in IDT meetings and care planning
โข Support quality and compliance standards
What You Bring
โข Active RN license (or compact eligibility)
โข Graduate of an accredited nursing program
โข Minimum 1 year of clinical nursing experience
โข Strong clinical judgment and compassionate care approach
โข Ability to manage a caseload independently
โข Excellent communication skills in sensitive situations
โข Basic EMR proficiency
โข Current CPR certification
โข Valid driver's license and reliable transportation
