Job description

We are looking for a dedicated candidate join our team as a Cognitive Behavioral Specialist (CBS) to service our patients in the Brunswick area. The Cognitive Behavioral Specialist (CBS) plays an integral role within our interdisciplinary care team by supporting patients experiencing anxiety, depression, and cognitive declineincluding dementia and mild cognitive impairment. The CBS provides structured behavioral interventions, promotes caregiver support, and aligns behavioral management strategies with medical treatment to
ensure a comprehensive, patient-centered care approach.

Cognitive Behavioral Specialist perform visits in facilities (ALF and ILF's) and patients' homes in their designated service area. You must have reliable transportation as travel is required daily. This is a full time, salary-based working 8-hour shifts (Monday-Friday; 8:00AM-5:00PM)

What Are We Looking For?

Your Health is currently looking for a Cognitive Behavioral Specialist to join our growing primary care family. A successful Cognitive Behavioral Specialist will be able to perform these essential duties and

The following is a list of essential functions, which may be subject to change at any time and without
notice. Management may assign new duties, reassign existing duties, and/or eliminate function(s)

Areas of Responsibility:

  • Manage a caseload of patients diagnosed with cognitive behaviors.
  • Conduct patient visits at a cadence tailored to patient needsinitially weekly, then biweekly, and eventually monthly, unless clinical needs dictate more frequent interaction.
  • Observe behavioral patterns across different times of day and settings; maintain detailed behavioral documentation.
  • Implement a structured behavioral protocol to assess and address behavior, identify emotional or environmental triggers, and support strategic behavioral interventions.
  • Monitor changes in behavior, function, and cognition to identify the need for care plan adjustments.
  • Work collaboratively with primary care and specialty providers to ensure alignment in behavioral and medical treatment plans.
  • Participate in interdisciplinary case reviews, care group meetings, and telehealth consults.
  • Educate and support caregivers in identifying, managing, and preventing behavioral triggers.
  • Facilitate caregiver coaching sessions focused on behavioral modification and communication strategies.
  • Participate in or lead education initiatives for families, caregivers, and the broader community
    on topics related to dementia, behavioral health, and aging.

Qualifications:

  • Minimum of five (5) years of experience working with individuals with dementia or cognitive impairments.
  • Experience in roles such as Activities Director, CNA, MA, RN/LPN, or other direct care professionals in geriatrics or memory care settings. If licensed, license must be in good standing.
  • Actively enrolled in or willing to enroll in a Dementia certification course.
  • Dementia Care certification preferred or willingness to pursue certification.
  • Familiarity with behavioral health medications and interventions for this population.
  • Ability to read and communicate effectively.
  • Strong written and verbal skills.
  • Basic computer knowledge.
  • Ability to manage and demonstrate effective leadership skills.
  • Should demonstrate good interpersonal and communication skills under all conditions and circumstances.
  • Ability to foster a cooperative work environment.
  • Team player with ability to manage multiple responsibilities and demonstrate sound judgment.
  • Must be able to work flexible hours and travel between offices, facilities, etc. Must be a licensed driver with an automobile that is insured in accordance with state and/or organizational requirements and is in good working order
