Job description

ABOUT THE ROLE

As a Registered Nurse at Daymark Health, you will be a key member of an integrated Care Team, which includes a Nurse Practitioner, Health Partner, Enrollment Specialist, and Social Worker. You will deliver hands-on, compassionate care directly in patients' homes and through virtual platforms. You will provide triage, case management, and care navigation for oncology patients, many of whom have additional comorbidities.

Your primary focus is delivering direct clinical care in the home: administering treatments, managing symptoms, conducting assessments, and being a trusted, present resource for patients and their families during one of the most challenging times of their lives. You will also support patients through virtual platforms and help them access Daymark's services and support.

WHAT YOU'LL DO

In this role, you will provide direct patient care in the home and through virtual platforms. Additionally, you will conduct health assessments, educate patients, and deliver high-quality care to meet each patient's unique needs. In this role you will:ย 

  • Deliver In-Home Clinical Care. Master hands-on care as a clinician in patients' homes administering IV fluids, accessing ports, conducting point-of-care testing, and managing acute and chronic symptoms with confidence and skill.
  • Provide Urgent Clinical Support. Triage urgent needs from patients, escalations to a home visit or to our Nurse Practitioners. Offer consultative clinical guidance and help patients navigate critical moments in their care.
  • Support Patients Across the Full Care Continuum. Provide comprehensive clinical support from active treatment through survivorship or end-of-life care, adapting your approach to meet each patient where they are.
  • Proactively Address Patient Needs. Quickly identify and act on patient needs: symptom management, nutritional support, discharge planning, and provider referrals, without waiting to be asked.
  • Address Social Determinants of Health. Partner with your non-clinical Care Team members to resolve barriers like food insecurity and transportation that affect patient outcomes.
  • Master Daymark Systems. Become familiar with Daymark's systems, tools, technology, and partners.
  • Build Trusting Relationships. Establish strong, trusting relationships with patients and their care networks by prioritizing empathy and active listening in every interaction.ย 
  • Adhere to Operational Standards. You're following Care Team policies, procedures, and documentation standards, contributing to operational efficiency and maintaining high-quality care with comfort.ย ย 

WHAT WE ARE LOOKING FOR

  • Experience: A Bachelor of Science Degree in Nursing is required, along with a compact unrestricted registered nurse (RN) compact licensure in Rhode Island. Additionally, you have at least 3 years of nursing experience, with some oncology nursing experience and/or home care experience strongly preferred. You are passionate about helping patients understand the benefits of palliative care.ย 
  • Availability: You can work with our team and patients full-time, five days per week.ย 
  • Qualifications: Oncology-related certifications such as Oncology Certified Nurse (OCN), Advanced Oncology Certified Nurse (AOCN), Advanced Oncology Certified Nurse Specialist (AOCNS), or Certified Case Manager (CCM) are a plus.
  • Strong Clinical Skills. You're comfortable administering IV fluids, accessing ports when needed, and conducting point of care testing.ย 
  • In-Home Care Competency: Comfortable and effective working independently in patients' homes, exercising strong clinical judgment without immediate on-site backup.
  • Comfort with Technology. You're comfortable using Slack, Zoom, Google Suite, and EHR systems.ย 
  • Strong relational skills. You have the ability to establish rapport and work effectively with diverse populations including health care providers, clinical staff, patients, and family members.ย ย 
  • Bias to action: You don't wait for someone to tell you what to do.ย  You know how to identify and prioritize your initiative's needs and do what it takes to ensure that urgent and important needs are acted on immediately.ย

Additional Information

The expected compensation for this role is $90,000 to $110,000 annually, depending on location & experience.
