As a Hospice Admissions RN You will:ย 

  • Visit patients (in homes, hospitals, nursing facilities, etc.) who are referred for hospice careย 
  • Explain hospice services, discuss goals and care with the patient and families, answer any questions, and obtain necessary consents and legal documentsย 
  • Obtain medical history, current medications, and symptomsย 
  • Perform comprehensive clinical assessments to determine eligibility for hospice based on Medicare/insurance guidelinesย 
  • Coordinate the start of services (nursing, aides, equipment, medications.)ย 
  • Collaborate with interdisciplinary teamย 
  • Maintain accurate and timely documentation ย

Qualifications
  • Graduate of an accredited nursing school with current licensure to practice in state of operationย 
  • One year of hospice nursing experience required, 3+ years preferred or similar setting such as Long-Term Care, Intensive care, or Emergency room will be considered in place of hospice nursing experienceย 
  • Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working orderย
