Director of Nursing (Clinical Director) – Home Health
Pay: $100,000.00 - $120,000.00 per year

Millennium Home Health is actively seeking a dedicated and experienced Registered Nurse (RN), licensed in California, to serve as our Director of Nursing (Clinical Director). This is a vital leadership position responsible for overseeing all clinical operations, ensuring regulatory compliance, and promoting high-quality patient care across our home health services.

Key Responsibilities

  • Direct, manage, and evaluate all clinical services delivered to home health patients
  • Provide leadership and guidance to field nurses, therapists, and clinical supervisors
  • Ensure compliance with all state, federal, and accreditation (ACHC) regulations
  • Oversee documentation practices, patient records, and quality standards
  • Participate in budgeting, staffing, and census planning with the Executive Director
  • Monitor clinical performance, conduct evaluations, and provide mentorship and support
  • Develop and implement training, orientation, and continuing education for staff
  • Serve as a clinical resource for care planning, implementation, and evaluation
  • Facilitate communication between patients, families, physicians, and the clinical team
  • Promote agency services and maintain relationships with referral sources and community partners

Qualifications

  • Active California Registered Nurse (RN) license
  • Bachelor of Science in Nursing (BSN) required; Master’s in Nursing or Healthcare Administration preferred
  • Minimum of 3 years of management experience in home health or a related healthcare setting
  • In-depth knowledge of home health regulations, ACHC/Joint Commission standards, Medicare and Medi-Cal
  • Excellent leadership, communication, and organizational skills
  • Proficiency in EMR systems such as Devero
  • Reliable transportation, valid California driver’s license, and auto insurance

Preferred Experience

  • Experience overseeing interdisciplinary teams (RN, PT, OT, MSW, etc.)
  • Familiarity with OASIS documentation, audits, and QAPI programs
  • Prior experience as a Clinical Supervisor or Director in a certified home health agency
  • Bilingual abilities (Spanish, Tagalog, or other languages) are a plus
