• Conduct comprehensive assessments of clients' psychosocial needs, integrating emotional, mental, and social factors to create tailored treatment plans.
• Provide individual and family counseling sessions, focusing on coping strategies, emotional support, and navigating life transitions.
• Collaborate with clients to develop and implement personalized care plans that reflect their individual goals and preferences.
• Advocate for clients' rights and empower them to access community resources essential for their mental and emotional health.
• Regularly monitor clients' progress through virtual check-ins, adjusting treatment plans as necessary to align with their evolving needs.
• Facilitate interdisciplinary meetings to coordinate with healthcare professionals on client care strategies and updates.
• Maintain precise and up-to-date documentation of client interactions, assessments, and care progress in compliance with healthcare regulations.

This role primarily focuses on adults and seniors who are navigating mental health challenges, including but not limited to anxiety, depression, adjustment disorders, and various life crises. Your empathy and interpersonal skills will be crucial in building rapport and trust with clients, helping them feel comfortable sharing their experiences and challenges. You will play an essential role in fostering resilience and promoting mental wellness within this population.

• Master's degree in Social Work from an accredited institution.
• Current and valid state licensure as a Licensed Independent Clinical Social Worker (LICSW).
• A minimum of two years of experience providing mental health services, preferably in a community setting.
• Comprehensive understanding of social work principles, mental health issues, and community resources.
• Exceptional skills in assessment, counseling, and problem-solving.
• Demonstrated ability to empathize and establish rapport with diverse client populations.
• Strong communication skills to effectively collaborate with interdisciplinary teams.
• Proficient in using electronic health records (EHR) and telehealth platforms to deliver effective care.

• Experience working in a remote or telehealth setting, which is advantageous for adapting to our clinic's virtual model of care.
• Experience with geriatric clients and specific knowledge related to aging-related challenges is preferred.
