Job Summary and Responsibilities
Schedule: Monday-Friday (Flexible - between the hours of 730a-5p)
**This position will be working remote after 2 weeks of IN PERSON training. MUST live within the Omaha Metro - Council Bluffs Location to be considered!**
The Virtual Discharge Clinic RN facilitates inbound and outbound telephonic and computer communication with clinics, hospitals, patients, providers and consumers to deliver patient care that promotes wellness and safety.
  • Discharge documentation, updating charts in the EHR system and coordinating follow-up appointments as needed
  • Reviewing discharge instructions ensuring the patient understands next steps
  • Medication review with patients as needed
  • Provides virtual patient care based on practice guidelines, standards of care, and federal/state laws and regulations.
  • Accepts inbound communication such as calls from patients, pharmacies and other health-care entities.
  • Performs outbound calls to notify patients of reviewed lab and imaging results according to protocols that require review of the patient chart to answer questions.
  • Performs telephone triage utilizing evidence based triage protocols.
  • Educates patients and families about health status, health maintenance, and management of acute and chronic conditions.
  • Follows medication refill protocols and acts accordingly to those protocols.
  • Ensures accurate patient documentation.

Job Requirements
Minimum Qualifications:
  • Current Registered Nurse (RN) license that allows you to practice in our state
  • Graduate of an accredited nursing program
  • Basic Life Support (BLS) from the American Heart Association required
