• Provide individual and group therapy sessions tailored to the unique needs of patients in a hospice setting.
• Utilize diverse clinical counseling techniques to address a spectrum of mental health issues effectively.
• Document patient progress accurately, including treatment plans and session notes, adhering to established medical documentation protocols.
• Facilitate case collaboration with interdisciplinary teams, ensuring that all patient interactions are cohesive and comprehensive.
• Maintain timely and thorough clinical documentation to support ongoing patient evaluation and treatment.
• Monitor and assess the psychosocial needs of patients and their families, offering appropriate interventions and support.

The LCSW will primarily serve hospice patients grappling with terminal illnesses and their families, offering emotional support and therapeutic interventions during an incredibly sensitive time. The ideal candidate will be empathetic, compassionate, and skilled in addressing grief, anxiety, and existential questions that arise in the hospice environment. A focus on helping clients achieve emotional closure and peace is paramount.

• A Master's degree in Social Work or a related field is required.
• A valid and current Licensed Clinical Social Worker (LCSW) license in Pennsylvania is mandatory.
• A minimum of 2 years of clinical experience in the mental health field, particularly in outpatient settings, is essential.
• Proficiency in electronic medical records and clinical documentation procedures is necessary to maintain compliance with regulations.

• Experience or training in motivational interviewing is highly desirable.
• Specialization in working with children and adolescents in a hospice context is a plus.
• Background in crisis intervention strategies and handling acute emotional distress effectively.
• Familiarity with gender issues and their impact on mental health is advantageous.
• Knowledge of community resources and referral networks specific to hospice and palliative care.
