ApogeeCare seeks experienced clinicians to conduct specialty geriatrics consults for older adults in the emergency departments (ED) via telehealth.

Position Overview

As a Telehealth Geriatrician - Site Lead with ApogeeCare, your expertise will drive improved clinical outcomes and enhanced patient experience. The ideal candidate will have a passion for improving care for older adults, the ability to provide patient-centered care in a virtual setting and the willingness learn and adapt to a new model of care delivery. This position requires multiple remote clinical shifts per week, allows administrative time and reports directly to the Medical Director. Key Responsibilities:

  • Serve as the primary point of contact for a Houston-based health system partner. Maintain knowledge of local resources and key stakeholders. Clearly communicate with health system partners, including but not limited to:

o Representing the organization in regular meetings with health system leadership

o Collaborating with partners to advance shared goals and strategic priorities

o Leading responses to partner feedback and driving follow-up actions

  • Perform telehealth geriatrics consultations to patients located at Houston-based ED, including the following key clinical tasks:

o Identifying and triage patients appropriate for specialty geriatrics consults

o Directing on-site ApogeeCare personnel to initiate and support virtual consults

o Identifying underlying geriatric syndromes and psychosocial complexities beyond the acute medical illness resulting in the ED utilization

o Developing care plans to facilitate safe dispositions and clearly communicating recommendations to ED providers

  • Develop strong working relationships with ED provider staff. Coordinate with other disciplines (e.g., case management, pharmacy, psychiatry, palliative care and hospice) to address complex patient needs and establish clinically appropriate care plans.

  • Document assessments and care plans in the electronic health record (e.g., EPIC). Maintain patient privacy as it relates to HIPPA and other relevant data security standards.

Qualifications:

  • Current licensure as a nurse practitioner (NP) or physician (MD/DO)

  • At least five years of experience in geriatric medicine

  • Board certification in geriatrics or gerontological nursing preferred

  • Strong clinical assessment and decision-making skills

  • Excellent communication abilities, both written and verbal

  • Comfort with telemedicine technology and virtual patient interactions

  • Ability to work collaboratively in a fast-paced environment
