Job Title: Registered Nurse (RN) - Home Health Case ManagerAbout the Facility
  • Home Health Services based out of Oakbrook

  • Field-based role providing care in patients' homes across designated territories

  • Collaborative and technology-enabled home care environment


Position Overview

We are seeking an experienced Home Health RN Case Manager to deliver high-quality, patient-centered care in a home setting. This role involves independent case management, clinical assessments, and coordination with interdisciplinary teams.


Experience Requirements
  • Minimum 2 years RN experience

  • At least 1 year in Home Health

  • First-time travelers: Considered based on experience


Patient Population
  • Chronic disease management: CHF, COPD, Diabetes

  • Foley catheter care

  • Wound care and wound management

  • PICC lines and infusion therapy

  • LVAD patients


Caseload & Productivity
  • Average 6 visits per day

  • Adjusted workload when completing multiple OASIS visits

  • Mix of routine visits and admissions


Required Certifications & Licensure
  • Active RN License

  • BLS (AHA)

  • Valid Driver's License

  • Proof of Auto Insurance

  • Reliable personal vehicle


Required Skills
  • Strong wound care and case management experience

  • Foley catheter management

  • PICC/central line care and dressing changes

  • Infusion therapy & port de-accessing

  • Comfortable working with interdisciplinary teams

  • Proficient in OASIS documentation

Scheduling & Work Structure
  • Shift: 8:00 AM until last patient is seen

  • Schedule: Monday-Friday (adjusted during weekend rotation)

  • Weekend Rotation: Every 3rd weekend

  • On-Call: None

  • Holiday Requirement: None

    • Option to work surrounding weekend days if taking holiday off

  • RTO: Up to 7 days with manager approval

  • Start from home → travel to first patient → continue visits

  • Participate in 8:00 AM virtual team huddles (via Microsoft Teams)

  • Patient Scheduling:

    • RN schedules follow-up visits

    • Scheduler assigns new patients


Orientation
  • New Employee Orientation (NEO)

  • Up to 2 weeks onboarding, including:

    • EPIC training

    • Preceptor support (with additional time if needed)


Support Structure
  • Manager and Supervisor available by phone

  • Home Health Aide support (separate visits)

  • Collaborative interdisciplinary team
