Job description

We are seeking qualified Nurse Practitioner that specializes in Wound Care to service our patients though out the Marietta/Gainesville area. As a Provider, you play a critical role in delivering exceptional patient care across various settings, including homes, facilities and telehealth. You are an integral part of the care team, collaborating with multiple healthcare professionals to ensure comprehensive and effective patient management. Your responsibilities include evaluating high-risk patients, coordinating care plans, and providing care management to improve patient outcomes.

Our providers conduct virtual patient visits from the comfort of their own homes as well as in-person visits in various settings including homes and facilities. This is a full-time salary-based position, with the opportunity to receive a monthly bonus.

The following shift(s) are available:

  • 8-hour shift (Monday-Friday; 8:00am-5:00pm)
  • 10-hour shift (Monday-Thursday; 7:00am-6:00pm)
  • 10-hour shift (Tuesday-Friday; 7:00am-6:00pm)

Your Health is currently looking for a Nurse Practitioner to join our growing primary care family. A successful Provider will be able to perform these essential duties and responsibilities. Areas of Responsibility:

  • Evaluate high-risk patients with complex medical needs and develop individualized care plans.
  • Coordinate care activities with other members of the interdisciplinary team to ensure continuity of care and optimal patient outcomes.
  • Communicate regularly with case managers, clinical coordinators, social workers, specialists, pharmacists, health coaches, and other team members to coordinate patient care.
  • Participate in interdisciplinary team meetings to discuss patient progress, treatment plans, and care coordination efforts.
  • Foster effective communication and collaboration to facilitate seamless patient management and interdisciplinary teamwork.
  • Meet staff efficiency expectations while providing high-quality care to patients in various settings, including homes, facilities, telehealth, clinics, and specialties.
  • Document care management activities accurately and efficiently to support patient care plans and regulatory compliance.
  • Provide patients with education and resources related to their conditions, treatment plans, self-management strategies, and preventive care measures.
  • Offer support and guidance to patients and their families to enhance their understanding of healthcare options and promote active participation in their care.
  • Adhere to all relevant healthcare regulations, policies, and procedures, including HIPAA and Medicare guidelines.
  • Monitor quality measures and gaps in care, identifying opportunities for improvement and implementing corrective actions as needed

Qualifications:

  • Nurse Practitioner (NP), Physician Assistant (PA), or Physician with a valid license to practice in the state of employment required. License must be in good standing.
  • Demonstrated experience in providing patient care, preferably in a home health, clinic, facility, telehealth, or specialty care setting.
  • Strong communication, collaboration, and interpersonal skills, with the ability to work effectively within an interdisciplinary team.
  • Ability to evaluate high-risk patients, develop care plans, and coordinate care activities to meet patient needs and improve outcomes.
  • Commitment to delivering compassionate, patient-centered care and promoting a culture of excellence in healthcare delivery.
  • Strong organizational and time management skills, with the ability to multitask and prioritize responsibilities effectively.
  • Ability to read and communicate effectively.
  • Strong written and verbal skills.

  • Ability to manage and demonstrate effective leadership skills.
  • Should demonstrate good interpersonal and communication skills under all conditions and circumstances.

  • Must be able to work flexible hours and travel between offices, facilities, etc. Must be a licensed driver with an automobile that is insured in accordance with state and/or organizational requirements and is in good working order.
