Overview

Office Location:   1028 S. Main Street Suite C Fond du Lac, WI 54935
Coverage area:   Travel is required throughout the Fond du Lac and South Territories

Schedule:   Full Time Monday-Friday 8AM-5PM

  

Generations Hospice is seeking a passionate, dedicated Social Worker to join our team! Our Social Workers provide emotional support, counseling, and resource coordination to patients facing terminal illness. This role plays a key part in the delivery of holistic care and ensures that patients' psychosocial needs are met with dignity and respect.
