Description
Adoray Home Health and Hospice is committed to providing the best home health, home rehab therapy, palliative and hospice care experience available with the BEST people. We are your local, not-for-profit, community-based health care agency providing quality care throughout the St. Croix River Valley of western Wisconsin.
We are looking for a full-time Social worker in western St. Croix, eastern Dunn, and all of Barron County to provide services to hospice patients and their families. The individual performs psychosocial assessment, interventions, planning; and implements and evaluates care. S/he is an active participant in the Hospice IDT. S/he also conducts home visits on home health patients as a social work assistant, under the supervision of the MSW. The Social Worker provides support and education to patients, families, staff, and the communities we serve. This position reports directly to the Support Services Manager and Bereavement Coordinator. Performance is measured by direct observation, communication, patient outcomes, documentation reviews, supervisory home visits, patient/caregiver communication.

Requirements
Minimum Qualifications
  • Bachelors of Social Work (BSW) degree from an institution accredited by the Council on Social Work Education.
  • One year of social work experience in a health care setting required, two years preferred.
  • Hospice and/or home health experience preferred.
  • Training or experience in bereavement or grief preferred, including assessment skills.
  • Understanding and commitment to hospice concept and philosophy.
  • Excellent communication skills.
  • Knowledge of community resources.
  • Knowledge of hospice philosophy.
  • Ability to work autonomously and make independent judgments.
