You are responsible for conducting virtual assessments, providing patient education, and triaging care needs, all while collaborating closely with physicians, specialists, and care coordinators through secure digital platforms. Regular team meetings, shared electronic health records, and real-time messaging tools help ensure seamless coordination and continuity of care. This collaborative approach is key to delivering high-quality patient outcomes in a virtual healthcare environment.
