Job Description
Working closely with the population health teams, primary care providers (PCP's), networked facility partners, and practice operations at Privia Health, the Health Advisor will provide care coordination and support for Privia's at-risk patients. They will provide inbound and outbound telephonic support for care management team. Primary work will include outbound calls to patients to provide coordination services, fill gaps in care, and promote improvements to patient outcomes.
  • Assist physician practices in identification and completion of quality care goals through direct patient outreach
  • Collaborate effectively with integrated care team to support care coordination for patients
  • Review patient non-compliance lists and conduct telephonic outreach to ensure timely disease specific and wellness visits
  • Answer and resolve business office inbound calls related to population health, care coordination, or other basic clinical activities
  • Motivate patients to be proactive about their healthcare
  • Assist patients in accessing care at the appropriate source
  • Make appropriate referrals to internal care team, community resources, and disease management programs
  • Develop relationships with the care coordination teams
  • Coordinate home care referrals
  • Assist in scheduling PCP or specialist appointments
  • Route clinical calls and referrals to the appropriate resource: Nurse care manager, pharmacist, social worker, or appropriate resource
  • Must comply with all HIPAA rules and regulations
  • Other duties as assigned

Qualifications
  • Prior experience navigating Electronic Medical Record systems (e.g., Epic, Athena, or similar platforms); willing to learn internal tools.
  • Background in patient outreach across diverse demographics (pediatrics to adults), including transition of care (post-discharge recovery, medication adherence, closing gaps in care, and patient advocacy).
  • Ability to work a hybrid schedule (3 days remote, 2 days field/in-office/in-practice)
  • Comfortable navigating new platforms, pulling/reviewing payer reports, and using in-house digital tools.
  • Ability to evaluate varied, complex patient cases, identify appropriate resources, and determine when to escalate cases to an RN versus managing them directly.

The salary range for this role is $21.00-$25.00hr in base pay and exclusive of any bonuses or benefits (medical, dental, vision, life, and pet insurance, 401K, paid time off, and other wellness programs). This role is also eligible for an annual bonus targeted at 10%. The base pay offered will be determined based on relevant factors such as experience, education, and geographic location.

In order to successfully work remotely, supporting our patients and providers, we require a minimum of 5 MBPS for Download Speed and 3 MBPS for the Upload Speed. This gives you an update as to how fast data transfer is with your internet connection and if it meets the minimum speed requirements. Work with your internet provider if you have questions about your connection. Employees who regularly work from home offices are eligible for expense reimbursement to offset this cost.
