This position is structured as a remote role, allowing for flexibility while maintaining a strong connection with clients. As a social worker in a school setting, you will engage with seniors who are in transition, often focusing on their mental health, community integration, and overall emotional wellness. You will collaborate with educational staff, caregivers, and community resources to ensure our clients receive comprehensive, quality care.

• Conduct individualized psychosocial assessments to identify seniors' emotional, mental, and social needs, considering factors such as mental health status and living conditions.
• Develop, implement, and monitor personalized care plans in collaboration with clients and their families, focusing on clear goals and actionable steps to support each individual’s journey.
• Provide one-on-one counseling and emotional support, assisting seniors in navigating life changes, such as relocation to assisted living facilities or coping with loss.
• Advocate for clients to ensure their rights are upheld and they have access to community resources, aiding in services such as financial assistance, healthcare, and social engagement programs.
• Collaborate with an interdisciplinary team, including physicians, nurses, and other health professionals, to enhance the care delivered to our seniors, holding regular case meetings to ensure a unified approach.
• Regularly monitor clients’ well-being through virtual home visits, following up on care plans, and adjusting strategies as necessary based on clients’ evolving needs.
• Educate families about available community resources and support systems, facilitating connections to local support groups and activities designed to foster socialization.
• Maintain accurate and timely documentation through an electronic health record (EHR) system, including treatment plans, session notes, and follow-up actions.
• Stay informed about changing policies and regulations in social work, ensuring best practices and compliance with ethical standards.

The primary focus of this role is on seniors, many of whom may be dealing with complex emotional and social challenges. Understanding their unique circumstances, including transitions such as entering retirement communities or dealing with bereavement, is crucial. You will liaise not only with the clients but also with their families, ensuring that all voices are heard and respected in the care process.

• A Master’s degree in Social Work from an accredited institution.
• Active and valid state licensure as a Licensed Clinical Social Worker (LCSW).
• A minimum of two years of direct experience as a social worker, preferably within a geriatric or community center environment.
• Comprehensive knowledge of social work principles, challenges facing the elderly, and relevant community resources.
• Strong counseling, assessment, and problem-solving skills.
• Ability to demonstrate empathy and build rapport with clients and their families.
• Excellent communication skills, both verbal and written, facilitating effective teamwork and collaboration.
• Proficiency in using EHR systems and general computer applications for documentation and communication.

• Experience working with interdisciplinary teams in care coordination.
• Familiarity with community-based programs for seniors, enhancing the ability to connect clients to support services.
• Experience within school settings emphasizing mental health and wellness.
