The Role

Help build the operational backbone of Circadia's growing Home Health program. As a Home Health Scheduler, you will coordinate daily schedules for our field-based nurse practitioners, ensuring they spend more time caring for patients and less time driving between visits. This role requires expertise in home health scheduling, geographic route planning, provider utilization, and real-time schedule management. Your work will directly impact patient access, provider efficiency, and our ability to scale home-based care.

What You'll Do Provider scheduling & route optimization
  • Build and optimize daily schedules for nurse practitioners conducting in-home or virtual visits

  • Coordinate geographically efficient routes to minimize travel time and maximize daily patient visits

  • Balance provider workloads across assigned territories

Patient coordination
  • Schedule new patient visits and follow-up appointments

  • Confirm appointment availability with patients and caregivers

  • Manage cancellations, reschedules, and same-day scheduling changes

Real-time schedule management
  • Monitor provider schedules throughout the day and quickly adjust for changes

  • Fill open appointment slots while maintaining efficient routing

  • Communicate scheduling updates with clinical staff and patients

Operational support
  • Collaborate with operations, nurse practitioners, and care teams to support seamless daily operations

  • Maintain accurate scheduling documentation within EMR and scheduling platforms

  • Identify workflow improvements that increase provider productivity and scheduling efficiency

Scaling the program
  • Help develop scheduling best practices as Circadia rapidly expands its Home Health program

  • Support onboarding of new providers by integrating them into efficient scheduling workflows


Must-Have Qualifications
  • 2+ years of scheduling experience in Home Health, Home Care, Hospice, or another mobile healthcare setting

  • Experience scheduling field clinicians traveling between patient homes

  • Strong understanding of geographic routing, travel optimization, and territory management

  • Experience managing frequent schedule changes and same-day adjustments

  • Excellent organizational, communication, and multitasking skills

  • High attention to detail and ability to work independently

  • Comfortable working in a fast-paced, rapidly growing startup environment

  • Proficiency with EMRs, scheduling software, Microsoft Office, and Google Workspace


Nice-to-Haves
  • Experience scheduling Nurse Practitioners or Registered Nurses

  • Familiarity with route optimization or mapping software

  • Spanish preferred
