IMMUNIZATION REQUIREMENTS (For Health Center and Direct Patient Care Positions)

For positions in a health center with direct patient care, vaccinations are required. Therefore, if you were to receive an offer and accept employment, we would require the following documents from you prior to your first day of employment. Please do not bring this documentation to your interview – you will have forms to complete that require the documentation should you be offered the position.

Hepatitis B vaccination records and titers (no titers required if NE/IA)

Measles, Mumps and Rubella (MMR) vaccination records or proof of immunization

Chicken Pox vaccination records or proof of immunization (Not required if NE/IA)

Tetanus shot documentation (Not required if NE/IA)

Tuberculosis PPD Skin Test that is no older than 12 months

BLS Certification (Only required for Health Center Associate II, Medical Assistant, Licensed Practical Nurse, Registered Nurse)

Pre-Employment Physical (Only required for SD positions)
