Job description

SUMMARY: Under the general direction of the Director of Continuity of Care, the RN, Case Manager- Discharge Planner, develops an appropriate discharge plan, making recommendations for alternative levels of care, identifying cost effective care options and facilitates appropriate clinical follow up. This work is done through patient assessment and engagement, chart review and collaboration with the healthcare team. The RN, Case Manager monitors patient progress throughout the hospitalization, modifying discharge plans as needed and continually assessing medical necessity. The RN, Case Manager provides information to post hospital vendors as needed to ensure ongoing recovery for the patient.
JOB REQUIREMENTS
Education: Graduate of an accredited school of nursing.
Licensure: Current Arkansas or Compact State Registered Nurse license in good standing.
Experience: Experience in previous Utilization Review or Case Management preferred.
Certifications: ACM or CCM Certification preferred
Other: Ability to read, write and communicate effectively. Basic computer knowledge required.
Preferred Education: Bachelor's Degree
