Registered Nurse (RN)

Requirements:

  • Active Registered Nurse License in the state of Virginia
  • Tuberculosis (TB) screening/QuantiFERON/Chest X-Ray (within 1 year)
  • Proof of U.S. work authorization (valid, unexpired documents: government issued ID)
  • Reliable transportation

Responsibilities:

  • Conducting Start of Care (SOC) visits to evaluate medical, physical, and emotional needs and creating individualized care plans.
  • Performing regular reassessments to monitor progress and adjust care plans as needed.
  • Providing patient and family education on condition management, medication adherence, and preventive health measures.
  • Collaborating with physicians, therapists, social workers, and families to ensure coordinated, comprehensive care.
  • Maintaining accurate clinical documentation in compliance with Verginia Board of Nursing regulations and agency policies.
  • Supervising and supporting Certified Home Health Aides (CHHAs) and other care team members.
