The Veteran's Communicare Therapeutic Clinic is actively seeking a dedicated Licensed Professional Counselor (LPC) to join our team in a fulfilling remote position. Our full-service clinic offers a variety of services including obstetrics, pediatrics, family planning, and dentistry. We prioritize addressing and managing social concerns, ensuring confidentiality in all treatments while accepting Medicaid, Medicare, and most major private health insurance policies. This position is entirely remote, allowing for flexibility while maintaining a strong commitment to client care. The work setting also incorporates an outpatient office environment where collaboration with healthcare team members occurs. You will have the opportunity to design your own schedule based around patient appointments, ensuring a balance between professional responsibilities and personal life.

• Collaborate effectively with primary care providers to identify and address mental health and psychosocial issues, serving as an integral member of the primary care team.
• Conduct thorough patient assessments focusing on diagnostic and functional evaluations, and provide actionable recommendations for treatment goals and plans.
• Deliver targeted interventions tailored to the unique needs of patients experiencing mental health challenges.
• Guide primary care providers to determine the appropriate level of care for patients, identifying those who would benefit from specialized mental health resources.
• Implement follow-up procedures to monitor patients' progress and adjust treatment plans as necessary.
• Develop relapse prevention plans when appropriate and support patients in maintaining stability in their daily functions.
• Identify at-risk patients and collaborate on prevention strategies to avert deterioration of their mental and physical health.
• Document patient interactions and progress meticulously in treatment charts, adhering to confidentiality standards.
• Maintain open communication with primary care and other healthcare providers, ensuring that everyone is informed of patient needs and treatment advancements.
• Assist patients with self-management skills and provide educational resources that empower them to engage actively in their treatment and recovery paths.
• Consult with clinic management and multidisciplinary team members regarding behavioral health services and participate in outcome evaluations and program assessments.
• Support patient adherence to medical treatment plans initiated by primary care providers, addressing medication side effects as needed.
• Strictly follow the Code of Ethical Conduct for Clinical Professionals and undertake other duties as assigned.

Your primary focus will be on serving veterans and their families, addressing their unique mental health needs and challenges. You'll work with a diverse clientele, adapting your approaches to cater to individuals coping with trauma, PTSD, anxiety, and other behavioral health issues prevalent within this community.

• A Master’s Degree in Clinical Counseling or a related field is required.
• A current Licensed Professional Counselor (LPC) License.
• Comprehensive knowledge of mental health assessments, treatment planning, and various therapeutic methodologies including family therapy and child psychotherapy.
• Understanding of DSM-IV diagnoses and behavioral therapy practices.
• Strong physical and mental health to meet the rigors of the position.
• Awareness of the societal impacts of mental and physical health issues on individuals and families.
• Familiarity with community resources to assist clients in navigating available support services.

• Bi-lingual proficiency in English and Spanish is preferred but not required.
• Prior experience working in outpatient settings or with veteran populations is a plus.
• Experience using electronic health record (EHR) systems for patient documentation and communication.

The position offers a competitive salary range of USD 87,000 – 105,000 per year, commensurate with experience.
