Introduction to the Role and Practice
Centra's PACE (Program of All-Inclusive Care for the Elderly) is a nationally recognized model of care serving adults aged 55 and older in our community. The mission of PACE is to deliver person-centered care in the setting that's most appropriate for each participant-whether that's in their own home, with family, in long-term care, or assisted living.
PACE is a capitated model of care, which means there are no productivity metrics, only a focus on delivering the care each participant truly needs. Everything is coordinated under one roof with a dedicated interdisciplinary team of providers, nurses, dietitians, physical and occupational therapists, social workers, activities coordinators, and transport staff.

Responsibilities
You Can Expect:
  • Monday - Friday 8:00am - 5:00pm - no weekends or holidays!
  • Dedicated team of providers, support staff, physical therapists, occupational therapists, dieticians, and social workers
  • 5 patients per day - small volume
  • Call is once every 7 weeks, 5pm - 8am, you are not expected to go out, calls are triaged before reaching you!
  • This opportunity is perfect for someone looking for meaningful relationships with our elderly population.

Qualifications
  • Graduate of an accredited Nursing Program, and Masters of Science Degree in Nursing (MSN)
  • 2 years of Nurse Practitioner experience
  • Current licensure by the Virginia State Board of Nursing as a Nurse Practitioner with prescriptive authority. Maintains BLS and ACLS Certifications. Hold a current, active license as a registered nurse in Virginia or hold a current multistate licensure privilege as a registered nurse. Nationally recognized certification in field of specialization
  • Experienced providers in palliative, hospice, primary care, or long-term care are encouraged to apply
