Job Title: SSVF Case Manager
Department: Veteran's Program
Reports To: Sr. Director of Supportive Services
FLSA Status: Exempt
Hours: Full Time
Summary
The Case Manager/Outreach worker works directly with veterans and their families by helping to outreach to the community and developing goal plans to meet both the veterans’ immediate needs housing needs, providing crisis intervention and resource referrals. Maintains computerized case management and hard copy files and is responsible for special projects as assigned.
Supervisory Responsibilities

Essential Duties and Responsibilities
  1. Assess new clients during screening, intake and orientation
  2. Assess program qualification criteria of client and aid in qualifying or referring to outside resources
  3. Assisting veterans in planning of housing attainment and sustainability including job training, education, money management income and benefits attachment
  4. Monitors household progress on housing stability plan
  5. Provide ongoing support, crisis intervention and housing stability support
  6. Coordinate housing, benefits, medical, legal, substance abuse services as needed
  7. Keep client files current in case file and database
  8. Conduct home visits to support client in housing stabilization
  9. Act as a liaison with VA outreach departments, community outreach departments, VASH department and the VA benefits and hospital departments
  10. Participate in conferences, workshops, special projects and staff meetings
  11. All other duties as assigned
Resident, Guest and Internal Customer Focus
SSVF Case Manager is sensitive to emotional, spiritual and practical needs of resident, families, guests and staff members in all interactions. Provides positive feedback and comments about the quality of work and effort that have been offered by residents, families and coworkers. Adjusts priorities and schedules to meet specific customer needs. Reports and/or corrects problems related to safety, cleanliness and comfort of resident and work areas. Universal Precautions and Infection Control techniques are followed. Understands own role facility safety plans and procedures related to fire, weather and other disasters is demonstrated.
Education and/or Experience
  • AA degree
  • Preference is given to Veterans
  • Knowledgeable of substance abuse/mental health issues, interventions and treatment planning
  • Some knowledge of affordable housing
  • Basic computer skills
  • Basic knowledge of community resources for Veterans
  • Ability to manage crisis situations
  • Must be 21 years of age
  • Personal car and valid Florida driver’s license
  • Acceptable annual Motor Vehicle Record check
  • Ability to pass a Level II Background Screening
