Job description

JOB SUMMARY:

The BPP Outpatient Value Based Care (VBC) Nurse is a member of the outpatient clinic team and practices in accordance with applicable policies, procedures, and standards established by the Governing Board and Administration of Beaufort Memorial. The focus of the VBC nurse is on improving patient health outcomes, reducing hospital readmissions, and lowering costs through proactive, high quality, patient-centered care and chronic disease management to our diverse population.

VBC nurses review documentation, close care gaps, manage transitions of care, assist with care planning, monitor for results, and utilize data to coordinate care for patient follow-up. Monitors patient care progress towards goals, communicates needs to the multidisciplinary team, and uses resources to optimize effective and efficient care across our care settings.

This position collaborates closely with the clinical nurse director to ensure coordinated initiatives meet value-based care program goals and patient care needs in the clinic.

JOB QUALIFICATIONS:

EDUCATION:

  • Graduate of nursing program (LPN, ADN, BSN, or MSN)

EXPERIENCE:

  • Demonstrates sound judgement and critical thinking in problem-solving situations.
  • 2-3 years previous hospital, physician practice, home health, case management or care coordination experience preferred.
  • The ideal candidate will work both independently and as part of a team.

JOB KNOWLEDGE, SKILLS & ABILITIES REQUIRED:

  • Knowledgeable about infection prevention control practices and universal precautions; basic knowledge of CPT and ICD-10 coding rules and regulations; knowledgeable about principles of patient confidentiality, privacy and safety. Knowledgeable of drugs/medications.
  • Strong critical thinking, proficiency in Electronic Medical Records (EHR) and Quality/VBC program electronic platforms.

CERTIFICATIONS/ LICENSES:

  • Current South Carolina RN or LPN licenses held in good standing.
  • BLS certification required prior to first clinical shift (must be issued by the American Heart Association)

ONGOING COMPETENCIES

  • Annual Review of Fire/Safety/Disaster Plan
  • Clinical skills review as assigned
