Join U.S.VETS and help veterans and their families overcome barriers to housing, employment, healthcare, and long-term stability. As a Case Manager II, you will provide comprehensive case management, advocacy, care coordination, and supportive services to veterans experiencing homelessness or housing instability.

This role is ideal for candidates with experience in social services, case management, housing, behavioral health, workforce development, or veteran services who are passionate about helping clients achieve independence and permanent housing.

Key Responsibilities
  • Manage a caseload of veterans and assess housing, employment, financial, medical, behavioral health, substance use, legal, and social service needs
  • Develop individualized action plans and housing stability goals with clients
  • Coordinate services with the VA, healthcare providers, community agencies, housing providers, and other partners
  • Facilitate client intake, eligibility verification, referrals, and discharge planning
  • Provide ongoing case management, advocacy, supportive counseling, and life skills education
  • Conduct home visits and wellness checks to support housing stability and client well-being
  • Connect veterans and families to housing, healthcare, employment, benefits, education, transportation, and community resources
  • Maintain accurate case notes, client records, assessments, service plans, and discharge documentation
  • Enter and maintain client information in HMIS and other electronic databases
  • Facilitate support groups, life skills classes, workshops, and specialized services as assigned
  • Monitor client progress and update individualized action plans regularly
  • Support crisis intervention, problem-solving, and coordination of services during urgent situations
  • Participate in case conferences, team meetings, trainings, and quality improvement activities
  • Maintain confidentiality and comply with HIPAA, VA, HMIS, grant, and organizational requirements
  • Complete required mileage, timekeeping, client expense, and program documentation
  • Drive company vehicles for client transportation and work-related activities as needed

Requirements

  • Bachelor's Degree required. Degree in Social Work, Psychology, Counseling, or other related social service field preferred
  • Strong oral and written communication skills
  • Professional manner and appearance
  • Ability to work independently and within a team
  • Ability to take direction
  • Ability to work effectively with diverse group of clients, staff, and community members
  • Computer proficiency in Microsoft Office 365 applications, email, and internet research
  • Valid driver's license required. Must meet company insurance requirements and complete a provided driver training course

Preferences

  • Two years of experience in social services or related field preferred
  • Experience working with homeless and/or veterans preferred
  • Certification in Substance Abuse Counseling preferred
  • Experience facilitating support groups preferred
