Responsibilities:
JOB SUMMARY
The Inpatient Case Management Supervisor is accountable and responsible for overseeing daily operations within assigned areas of the department. Position oversees case management staff to include RN Case Managers, SW Case Managers and Case Management Assistants. Responsibilities include monitoring the activities, performance and development of staff members and the development and evaluation of new processes and improvements. The Supervisor conducts first level problem solving with staff, ensures schedules are completed and scheduling issues resolved, and elevates appropriate issues to the senior case management leaders. The Supervisor is responsible for ensuring adherence to standard work and resolving barriers to completing standard work. Position is also responsible for participating in interviewing and hiring process, facilitating successful onboarding and orientation, and providing input to resolve performance issues within assigned teams.
ESSENTIAL FUNCTIONS
  • Supervises, in conjunction with Case Management leadership, assigned personnel in the performance of their duties.
    Participates in and makes recommendations with regard to the interviewing and hiring of staff.
    Assigns and directs duties of staff.
    Is responsible to ensure that work of subordinate staff is done appropriately according to departmental standards and administers first line redirection or disciplinary action as necessary.
    Participates, observes, and documents performance and compliance with established standards in the performance appraisal process.
    Participates in both short-term and long-term staffing determinations.
    Assists Case Management leadership in higher-level disciplinary process and development of related Performance Improvement Plans.
  • Reviews patient/customer/employee complaints for issues and identifies trends.
    Investigates further as appropriate and provides recommended resolution.
  • Assists in the development, implementation, and evaluation of unit-specific programs and process improvement initiatives, which are designed to maintain staff skill levels.
  • Supports and facilities staff involvement in programs, committees, and encourages staff to promote a professional environment.
  • Fosters collaborative partnerships that involve all disciplines within Case Management with medical staff and members of all related system departments, especially Nursing.
  • Serves as first point of escalation for problem solving and barrier resolution. Ensures standard work adherence and assists with change management.
  • Develops internal processes and identifies process improvement needs. Monitors process improvement initiatives and institutes remedial action when needed.
    Disseminates data to staff to support standard work and process improvement.
    Provides recommendations on modifications to standard work when needed using a data driven approach.
  • Provides back up case management coverage in the hospital when needed.
  • Other duties as determined by senior case management leadership to support department needs.

EDUCATION
  • Bachelors Nursing Required or
  • Masters Social Work Required
  • Masters Nursing or other healthcare profession Preferred

EXPERIENCE
  • 2 years, Inpatient case management or inpatient medical social work discharge planning and evidence of informal or formal leadership within the team Required

LICENSE AND CERTIFICATIONS
  • Registered Nurse (RN) - License, or Licensed Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) Required
  • Certified Case Manager - Certification, in related field, e.g. case management certification Preferred
