• Serve as a primary liaison with families and outpatient providers, ensuring coordinated care and support tailored to each client's needs.
• Engage with insurance companies to manage pre-certification and utilization reviews, ensuring seamless access to necessary services for clients.
• Contribute to discharge and aftercare planning, guiding clients through transitional phases of their care journey.
• Facilitate therapeutic meal support sessions, utilizing these interactions to model healthy relationships with food and body image.
• Provide empathetic, affirming, and culturally competent care that acknowledges and addresses each client’s intersecting identities.

The ideal candidate will work with a diverse clientele, ranging from children to older adults, many of whom face unique challenges related to their mental health. This position requires sensitivity to various cultural backgrounds and experiences as you provide individualized support. • Master’s degree in Social Work from an accredited institution, providing a solid theoretical background in social work practices.
