Job description

Physical Therapist - Physical Therapy

Schedule: Shift: 8 hours

Assignment Details:

  • Contract Length: 14 weeks
  • Guaranteed Hours: 40

Requirements:

  • Current State License as a Physical Therapist
  • Proof of current CPR certification
  • Current driver's license. Must have reliable transportation and insurance
  • 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting
  • Previous home care experience - Preferred
  • Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program

Roles & Responsibilities:

  • Conduct clinical assessments to identify patient needs and determine appropriateness for physical therapy.
  • Document and complete Plans of Care (POC), progress notes, and other clinical records on an ongoing basis, revising as necessary.
  • Design and implement treatment plans for neuromuscular, neurological, and orthopedic disorders.
  • Provide education to patients and families regarding therapeutic procedures tailored to patient needs.
  • Oversee the performance of other team members in delivering patient care consistent with individual patient goals.
  • Implement infection control protocols and universal precautions in all patient care activities.

Compliance / Notes:

  • Must meet mandatory continuing education requirements.
  • Flexible schedule and local travel may be required.
  • Ability to perform physical tasks including lifting, bending, and prolonged standing.
  • Must read, write, and speak fluent English.
