• Conduct in-depth mental health assessments for hospice patients, focusing on psychosocial evaluations and risk assessments to identify their unique needs and challenges.
• Collaborate with patients and their families to develop individualized treatment plans, considering cultural factors and personal preferences in the care process.
• Implement evidence-based therapeutic interventions, such as cognitive-behavioral therapy (CBT), dialectical behavior therapy (DBT), and solution-focused brief therapy (SFBT), tailored to the patient's end-of-life experience.
• Provide vital psychoeducation to patients and families, equipping them with knowledge and skills to manage emotional distress and navigate difficult decisions associated with hospice care.
• Maintain precise clinical documentation in compliance with regulatory guidelines, ensuring continuity of care through accurate progress notes, treatment plans, and discharge summaries.
• Engage in crisis intervention as needed, collaborating with other healthcare providers to stabilize patients in acute distress and facilitate appropriate care access.
• Regularly evaluate treatment effectiveness through outcome assessments, adjusting treatment plans to ensure each patient receives the highest standard of care.
• Stay abreast of emerging trends and research in mental health, integrating new insights into clinical practice to enhance patient outcomes.
• Adhere strictly to ethical guidelines, ensuring the confidentiality and privacy of patient information at all times.

This position focuses on serving hospice patients and their families, who often face complex emotional, psychological, and social challenges related to end-of-life care. • A Master's degree in Social Work is required.
• An active State license as a Licensed Clinical Social Worker (LCSW) is mandatory for this role.

• Strong interpersonal and communication skills, particularly in establishing rapport with patients from a variety of cultural backgrounds, are necessary to succeed.

• The annual salary for this position ranges from $70,000 to $100,000, with potential adjustments based on experience, education, and location.
