Summary:
Responsible for coordination of the patient admission experience into a Skilled Nursing Facility, including assessment and documentation of patient status.
Responsibilities:
Coordinates the admission process from the point of patient arrival to the facility for care. Ensures a quality transition for patients and families.
Reviews hospital discharge summaries and coordinates with the admitting physician to complete the medication reconciliation and set up initial admission orders.
Assesses patient's condition, including biophysical, psychosocial and age-specific needs.
Ensures facility-specific admission paperwork and consents are signed, complete, and accurate.
Documents patient's status according to unit specific procedure.
Coordinates with the patient and interdiscipinary team to create a baseline plan of care and implements the nursing plan of care on assigned patients.
Initiates a patient education plan according to the individualized needs of the patient and as prescribed by the physician and/or policy; includes patient and family verbal and/or written instructions, as needed or requested.
Provides report and handoff to the unit staff once admission process is complete.
Maintains educational requirements as required by the Board of Nursing.

Licensure/Certification Requirements:
โ€ข Must have NC RN Licensure.

Professional Experience Requirements:
โ€ข Prefer one year of experience in skilled nursing facility. Epic eMAR experience preferred.
Knowledge/Skills/and Abilities Requirements:
โ€ข Language Skills: Ability to read, analyze, and interpret clinical information and technical procedures

โ€ข Ability to effectively communicate information and respond to questions from patients, physicians, family members, and other UNC Health Rex staff
