Job description

Are you an experienced Registered Nurse with a background in home health who enjoys working independently and making a meaningful impact in the community? This long-term opportunity as a Home Health Nurse Assessor allows you to conduct in-home assessments that help ensure patients receive the care and services they need.

Minimum Requirements

Candidates must have at least 2 years of home-based healthcare experience within the last 10 years. Private Duty Experience is accpted.

Must have NYS Driver's license or NYS Id
