Job description

Rehabilitation Therapy Position

Any traveler not currently in possession of an active State of HI license must apply upon acceptance of offer. Candidate must have Hawaii License by start of assignment. Responsibilities vary based on discipline, licensure, and scope of practice. Employees are expected to perform only those duties permitted under applicable state law and professional practice standards.

General Job Duties:

  • Provide rehabilitation services in a Skilled Nursing Facility (SNF) and/or Intermediate Care Facility (ICF)
  • Perform evaluations, reassessments, treatments, and other clinical services consistent with licensure, discipline, and scope of practice.
  • Develop, implement, or carry out individualized treatment plans in collaboration with supervising therapist, physician, and interdisciplinary team, as appropriate to the role.
  • Implement evidence-based therapeutic interventions to maximize function, independence, communication, mobility, swallowing, and quality of life, as applicable to the discipline.
  • Educate residents, families, and caregivers regarding treatment plans and patient progress.
  • Complete accurate, timely, and compliant documentation. Documenting progress, changes in plan of care, and discharges according to facility, state, federal, and payer requirements.
  • Supervise therapy assistants, when applicable to role.
  • Must be available to work Hawaii State and Federal Holidays as necessary based on facility operations.

Required skills and minimum years of experience:

  • Minimum two (2) years of experience in the SNF/ICF setting preferred.

Specific licensures and certifications:

  • Current Basic Life Support (BLS) certification.
  • Current State of Hawaii license in good standing appropriate to the discipline.

Shift of scheduled requirements:

  • Generally scheduled five (5) days/week but likely to include one weekend day.
  • Possible floating to other units or facilities based on census and staffing needs.

Training:

  • Orientation to the rehabilitation department, EMR documentation (Net Health Rehab Optima & Epic), and facility workflows provided.
