Prime Staffing is seeking a travel nurse RN Case Management for a travel nursing job in Mission Viejo, California.

Minimum Qualifications

  • Active California Registered Nurse (RN) License
  • Minimum of 2 years of recent Acute Care Case Management experience
  • Demonstrated experience performing:
    • Care Coordination
    • Discharge Planning
    • Utilization Review
    • Resource Management
    • Interdisciplinary Team Collaboration
  • Previous Travel Nursing experience required (First-time travelers will not be considered)
  • Strong knowledge of acute care workflows and patient care transitions.ย 

Key Responsibilities

  • Coordinate patient care throughout the hospitalization and discharge process.
  • Collaborate with physicians, nurses, social workers, and ancillary departments to develop and implement individualized care plans.
  • Facilitate timely and appropriate patient discharges and transitions of care.
  • Assess patient needs and identify barriers to discharge.
  • Perform utilization management and resource coordination activities.
  • Ensure compliance with hospital policies, regulatory requirements, and quality standards.
  • Advocate for patients and families to ensure optimal care outcomes.
  • Maintain accurate and timely documentation within the electronic medical record system.
