AsTransplantCoordinator,you will oversee and manage alllivertransplant patient activities at Weston Hospital.You willassistfellow caregivers in creating care plans, reviewing medical information, assembling test results,coordinatingtransplantsand more, all to support the exceptional experience ourcommunityhascome to expect. This role helps change the lives of patients and their families every day.

A caregiver in this position worksdays from8:00 a.m.- 5:00 p.m., with on-call requirements.

A caregiver who excels in this role will:

• Assess patients in the outpatient clinic along with the transplant physician.

• Facilitateplansofcare and symptom management with outpatient caregivers.

• Coordinate scheduling with the administrative coordinator for follow-up appointments.

• Review withthe transplant physician medical information from referring physicians andinitiateappropriate communication.

• Coordinate consultation with other disciplines and obtain follow-up information consult.

• Coordinate with the hospital unit personnel in the discharge of the transplant patientsand donors.

• Assistin obtaining medications and supplies from theappropriate sourceaccording to the patient's insurancecoverage.

• Contact referring physiciansand arrangefollow up in the local area or at CCF depending upon patientsand their activity.

• Prior to transplant, coordinate withthepatient's transplant physicianand referring physician inevaluating the patientfor the transplant candidacy.This includesfacilitatingtissue typing of recipient and donor, arranging the timing and collection of pretesting prior to transplant with referring physician and transplant team.

• Assemble test results of evaluation and arrangefurther testing ifindicatedafter discussion with transplant physicians.

• Serve ascontactperson for pre-transplant evaluation for physician and/or insurance companies.

• Prepare and present casestotheSelection Committee.

• Place patientsonthenational waiting list for transplantation.

• Coordinate ongoing care of inpatientsand outpatients waiting for transplant.

• Triage patients during the entire transplant process for any medical andpsychosocialissues.

• Beavailable by beeper and by phonein order toprovide immediate assessment and delivery of care ina timelyandcost-effectivemanner.

• Collaborate with Office Coordinators and Financial Counselorsin obtaining insurance clearanceforpatients prior to transplant procedure.This includes reviewing clinical information and protocols sent to insurance companies for review of medical necessity, and discussions with case managersregardingthe protocols and medical information.

• Coordinate informational packets, which are sent to patients prior totransplant.

• Review information with patientsupontheir arrivaland answerany questionsregardingthe transplant information and transplant schedule.

• Coordinate and provideeducationalsessions for patients, families, donorsprior to transplant.

• CollaboratewiththeOrgan Procurement Organization tofacilitatethe organprocurement surgery andsubsequenttransplant surgery, including notifying all personnel involved in the process.

• Facilitate procedures.

• Provide in-servicesfor nursingcaregivers andpresentations for any outside organization or group inquiring about the transplantation process.

Minimum qualifications for the ideal future caregiver include:

• Basic Life Support (BLS)Certification through the American Heart Association (AHA)or American Red Cross

• Twoyears of RN experience

• One year of acute caredisease-relatedhospital experience

Preferred qualifications for the ideal future caregiver include:

• Bachelor'sDegree

• Transplant or GI experience

• ICU experience

• Three or more years of RN experience

• Endoscopy background
