For an experienced RN drawn to advanced heart failure and transplant medicine, this is an opportunity to practice at the center of one of healthcare's most meaningful patient journeys. As a Post Heart Transplant Coordinator, you will help patients navigate life aftertransplantwith the clinical judgment, precision, advocacy, and relationship-based care that define exceptional transplant nursing.
UVA Health is seeking a mission-driven RN to join arapidly growing Heart Transplant Programwithin a highly collaborative academic medical center environment. This role supports adult post-heart transplant patients in the outpatient setting, partnering closely with APPs, physicians, surgeons, inpatient and outpatient teams, and caregivers to promote continuity, safety, and long-term success after transplant.
This role isMonday through Friday, 8:30 AM to 4:30 PM, with clinic coverage early in the week and a full clinic day on Wednesday.The first six months requireonsitepresence to support a structured orientation and deep transplant-specific onboarding,with potential hybrid/work-from-home flexibility after successful completion of orientation.
The Role:
As a Transplant Coordinator, you will serve as a central clinical resource for patients after heart transplantation, helping coordinate complex care needs across the transplant continuum.
This role is especially well suited for an RN with transplant experience, heart failure experience, cardiac nursing experience, ora strong desireto build a long-term career in advanced transplant care. The ideal candidate bringsexceptional attention to detail, strong care coordination skills, proactive patient advocacy, emotional intelligence, adaptability, and a genuine passion for transplant medicine.
In an average day, you may support clinic flow, coordinate follow-up care,monitorfor post-transplant concerns, communicate with patients and caregivers, and collaborate with a multidisciplinary team to ensure the right interventions happen at the right time. The work is complex, purposeful, and deeply patient-centered, with the opportunity to buildlifelong relationships with transplant recipientsas they move through their post-transplant journey.

MINIMUM REQUIREMENTS
Education: Bachelor of Science in Nursing from an accredited nursing program required.
Experience: 2 years of relevant experience.
Licensure: Licensed to Practice as a Registered Nurse in the Commonwealth of Virginia. American Heart Association (AHA) Health Care Provider BLS certification required. Relevant specialty board certification strongly preferred.
PHYSICAL DEMANDS
Job requires standing for prolonged periods, frequently traveling, bending/stooping. Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull 20 - 50lbs. May be exposed to chemicals, blood/body fluids and infectious disease.
The pay range for this role is $94,952.00 - $125,465.60 annually. Individual compensation will be determined by the selected candidate's qualifications, previous work experience, and/or education.Benefits
