Primary Work Location: Natalie's Placeย ย 

Natalieโ€™s Place is one of several Cardea Health transitional housing programs for medically complex and behaviorally vulnerable unhoused individuals, located in Fremont. Cardea Healthโ€™s transitional housing programs collaborate with a wide range of referring partners across Alameda County to support individuals with complex health and social needs. Natalieโ€™s Place provides comprehensive supportive services, including onsite nursing and caregiving support, transportation to medical appointments, nutritious meals, housing navigation and placement assistance, and additional wraparound services designed to promote stability, recovery, and long-term housing success.

Job Summary:ย ย 

The Program Manager of Transitional Housing will supervise and direct personnel and program activities for Cardea Health Transitional Housing Programs, including Natalieโ€™s Place, a recuperative care program serving people who have experienced homelessness. The Program Manager is accountable for assisting with managing facilities, clinical personnel, housekeeping and caregiving functions, food service, transportation, and housing navigation. In addition, the Program Manager will work with the nursing team to manage the flow of clients through the program, from admission to discharge.

Job Duties:ย 

  • Assist the Program Director of Transitional Housing and Director of Nursing for Transitional Housing with leading a team of dedicated clinical and non-clinical professionalsย ย ย 

  • Under the direction of the Program Director, oversee facilities and operations at a site of unitsย ย ย 

Support the Program Director in manage personnel and operations related to:ย  ย 

  • Clinical staffย ย 

  • Case managementย ย 

  • Housekeepingย ย ย 

  • Caregivingย ย ย 

  • Food serviceย ย ย 

  • Transportationย ย ย 

  • Housing navigationย ย ย 

  • Troubleshoot issues as indicatedย ย ย 

  • Escalate challenges when appropriateย ย ย 

  • Manage client experience in the program from referral to dischargeย ย ย 

  • Assist with data reporting as required for assessment of program performanceย ย ย 

  • Maximize program efficacy in securing service connections and permanent housing for clientsย ย ย 

  • Ensure that clients move through program in accordance with program parametersย ย 

  • Fill all beds promptlyย

Minimum Required Skills & Qualifications:ย 

  • Bachelorโ€™s degree or equivalent professional experienceย ย ย 

  • At least two years of related experience requiredย ย 

  • Proven experience as a team leader and operations professionalย ย ย 

  • Experience working with people experiencing homelessness preferredย ย 

  • Experience working in healthcare preferredย ย ย 

Physical Requirements:

  • Occasional sitting or stationary workย 

  • Frequent standing, walking, kneeling, stooping, and ascending stairsย
