Job description

St. Mary's Medical Center is seeking a casualDischarge Planning Specialist. The Discharge Planning Specialist (DPS) will assist the Care Facilitator and or Social Worker with the discharge planning responsibilities, to promote timely and safe discharge services. Under the supervision of the Care Facilitator and or Social Worker, the Discharge Planning Specialist facilitates and coordinates discharge services such as home health, DME, Rehab placement, and other community resources or agencies.

Education Requirements:

High School Diploma is required. Junior College/Technical School preferred.

Experience:

Education and training in hospital healthcare transitional care, and discharge planning beyond the hospital setting, preferred.

Certifications/Skills:

BLS required. Working knowledge of medical terminology. Excellent communication skills-verbal and written ability to work with patients, families, physicians, and other hospital departments.
