Hourly Rate: $18.70 -$21.63

Job Description:ย  Provides a broad range of social services to consumers and their families. Participates in treatment team planning and performs crisis intervention.ย  Assists in dealing with personal and social problems.ย  Serve as a liaison for social services.ย  Performs case management duties, education and skills building, Performs lead worker functions. Additionally, has contact with clients prior to release from inpatient facility, jail, or prison. Establish a connection with clientโ€™s supportive contacts. Transition planning activities include educating the individual and identified client supports on service options offered by the agency, participating in facility treatment team meetings to develop a transition plan, and making collateral contacts with other agencies and community resources when indicated. Will conduct screenings and necessary assessments to engage the individuals in services.

Job Responsibilities:ย  Will work Monday โ€“ Thursday @ 10-hour days.

Competencies: Knowledge of diagnostic impressions or criteria. Knowledge of how client advocacy system operates. Knowledge of individual and group therapy techniques. Knowledge of various therapeutic models and intervention as directly related to area of assignment. Knowledge of crisis intervention techniques.

Required Minimum Qualifications: Must be at least 21 years old to apply. Must possess a valid driverโ€™s license and have a clean driving record with less than 6 points. HS Diploma/GED and three yearsโ€™ experience in social services. Bachelor's degree with minimum one year experience preferred. Preference for experience with or knowledge of trauma informed, resilience-oriented care.
